Insurance and Billing Policy
First Choice Medical Group participates in most major insurance plans and government programs, including Medicare and TRICARE. Please verify your eligibility for our services with your insurance provider. If you have any questions about your healthcare coverage, you should first contact your insurance provider at the phone number reflected on the back of your insurance card or speak with someone in the benefits office where you work; or you can contact us to verify coverage prior to your appointment.
Important Notes About Your Insurance Coverage for Appointments at First Choice Medical Group
We bill your insurance plan as a courtesy.
Please note that you are still responsible for your bill if your insurance company does not pay your claim, or does not process it within a reasonable amount of time.
Some insurance plans have exclusions or exceptions that could apply to you. For example, there may be limits on the number of therapy visits you are permitted per year, or your plan may not pay for a surgical assistant or physician assistant who helps the surgeon during your operation.
While being treated at First Choice, please notify our front desk of any changes to your insurance coverage.
